Transparence clinique : illusion ou r\u00e9alit\u00e9 ?<\/strong><\/span><\/p>\n<div class='summary'>\n<div class='summary-article-content'>\n<p><a name='_Hlk202778485'><span style='color:Black;'><span style='font-family:;' lang='NL'><strong>Messages cl\u00e9s<\/strong><\/span><\/span><\/a><\/p>\n<ul>\n<li><span style='font-family:;' lang='FR-BE'>Une enqu\u00eate du <\/span><i><span style='font-family:;' lang='FR-BE'>British Medical Journal<\/span><\/i><span style='font-family:;' lang='FR-BE'> (BMJ) a r\u00e9v\u00e9l\u00e9 de <strong>graves irr\u00e9gularit\u00e9s<\/strong> dans les essais cliniques ayant soutenu l\u2019autorisation de mise sur le march\u00e9 (AMM) du ticagr\u00e9lor.<\/span><\/li>\n<li><span style='font-family:;' lang='FR-BE'>Les manquements concernent diff\u00e9rents aspects\u00a0: la m\u00e9thodologie&#x002C; le r\u00f4le des chercheurs et la conduite des essais.<\/span><\/li>\n<li><span style='font-family:;' lang='FR-BE'>Malheureusement&#x002C; ce n\u2019est pas un cas isol\u00e9. Le CBIP tient \u00e0 souligner l\u2019importance de la transparence des \u00e9tudes cliniques<\/span><span style='font-family:;' lang='NL'>.<\/span><\/li>\n<\/ul>\n<\/div>\n<\/div>\n<h2><span lang='FR-BE'>Introduction\u00a0<\/span><\/h2>\n<p><span style='font-family:;' lang='FR-BE'>Les \u00e9tudes cliniques&#x002C; en particulier les \u00e9tudes randomis\u00e9es contr\u00f4l\u00e9es (RCT)<\/span><ins cite='mailto:Camille%20Bertrand' datetime='2025-11-03T15:16'><span style='font-family:;' lang='FR-BE'>&#x002C;<\/span><\/ins><span style='font-family:;' lang='FR-BE'> constituent la \u00ab\u00a0mati\u00e8re premi\u00e8re\u00a0\u00bb de <\/span><i><span style='font-family:;' lang='FR-BE'>l\u2019evidence based medicine<\/span><\/i><span style='font-family:;' lang='FR-BE'> (EBM). Leur transparence et leur fiabilit\u00e9 sont donc essentielles.<\/span><\/p>\n<h2><span style='font-family:;' lang='FR-BE'>Le cas du ticagr\u00e9lor<\/span><\/h2>\n<p><span style='font-family:;' lang='FR-BE'>Depuis quelques mois&#x002C; plusieurs g\u00e9n\u00e9riques du <strong>ticagr\u00e9lor<\/strong>&#x002C; antiplaquettaire largement prescrit dans le syndrome coronarien aigu&#x002C; sont disponibles sur le march\u00e9. A cette occasion&#x002C; le <\/span><i><span style='font-family:;' lang='FR-BE'>BMJ<\/span><\/i><span style='font-family:;' lang='FR-BE'> a r\u00e9examin\u00e9 les donn\u00e9es scientifiques initiales ayant soutenu son AMM. L\u2019enqu\u00eate du BMJ r\u00e9v\u00e8le de <strong>graves irr\u00e9gularit\u00e9s<\/strong> dans les \u00e9tudes ayant soutenu son AMM&#x002C; toutes sponsoris\u00e9es par le fabricant.<\/span><br \/><span style='font-family:;' lang='FR-BE'>Dans un article du <\/span><i><span style='font-family:;' lang='FR-BE'><strong>BMJ<\/strong>\u00a0<\/span><\/i><span style='font-family:;' lang='FR-BE'>publi\u00e9<\/span><i><span style='font-family:;' lang='FR-BE'>\u00a0<\/span><\/i><span style='font-family:;' lang='FR-BE'>en d\u00e9cembre 2024<\/span><span style='color:#00B050;'><span style='font-family:;' lang='FR-BE'><strong><sup>1<\/sup><\/strong><\/span><\/span><span style='font-family:;' lang='FR-BE'>&#x002C; un senior editor du BMJ soul\u00e8ve de <strong>s\u00e9rieuses pr\u00e9occupations<\/strong> concernant l\u2019int\u00e9grit\u00e9 des donn\u00e9es de l\u2019\u00e9tude PLATO<\/span><span style='color:#00B050;'><span style='font-family:;' lang='FR-BE'><strong><sup>2<\/sup><\/strong><\/span><\/span><span style='font-family:;' lang='FR-BE'>. L&rsquo;\u00e9tude PLATO est l&rsquo;\u00e9tude pivot de phase III qui a conduit \u00e0 l\u2019AMM du ticagr\u00e9lor. Elle a \u00e9t\u00e9 publi\u00e9e en 2009 dans le <\/span><i><span style='font-family:;' lang='FR-BE'>New England Journal of Medicine<\/span><\/i><span style='font-family:;' lang='FR-BE'> (NEJM) (voir\u00a0<\/span><a href='https:\/\/www.cbip.be\/fr\/articles\/2047?folia=2031'><span style='font-family:;' lang='FR-BE'>Folia de juillet 2012<\/span><\/a><span style='font-family:;' lang='FR-BE'>).<\/span><\/p>\n<p><span style='font-family:;' lang='FR-BE'>Dans un article du BMJ publi\u00e9 en juin 2025<\/span><span style='color:#00B050;'><span style='font-family:;' lang='FR-BE'><strong><sup>3<\/sup><\/strong><\/span><\/span><span style='font-family:;' lang='FR-BE'>&#x002C; le m\u00eame senior editor du BMJ a exprim\u00e9 des inqui\u00e9tudes concernant l\u2019int\u00e9grit\u00e9 de deux autres \u00e9tudes sur le ticagr\u00e9lor. Il s\u2019agit des \u00e9tudes ONSET\/OFFSET et RESPOND&#x002C; dans lesquelles l\u2019effet inhibiteur du ticagr\u00e9lor sur l\u2019agr\u00e9gation plaquettaire a \u00e9t\u00e9 examin\u00e9. Ces \u00e9tudes ont \u00e9t\u00e9 publi\u00e9es respectivement en 2009 et 2010 dans la revue <\/span><i><span style='font-family:;' lang='FR-BE'>Circulation<\/span><\/i><span style='font-family:;' lang='FR-BE'>.<\/span><\/p>\n<p><span style='font-family:;' lang='FR-BE'>Dans ces \u00e9tudes&#x002C; plusieurs manquements ont \u00e9t\u00e9 mis en \u00e9vidence par le <\/span><i><span style='font-family:;' lang='FR-BE'><strong>BMJ<\/strong><\/span><\/i><span style='font-family:;' lang='FR-BE'>\u00a0:<\/span><\/p>\n<ul>\n<li><span style='font-family:;' lang='FR-BE'><strong>Probl\u00e8mes li\u00e9s aux chercheurs<\/strong>\u00a0: un chercheur ayant activement particip\u00e9 \u00e0 l\u2019\u00e9tude n\u2019a jamais \u00e9t\u00e9 mentionn\u00e9 comme auteur&#x002C; tandis qu\u2019un autre&#x002C; pourtant cit\u00e9 comme auteur&#x002C; a d\u00e9clar\u00e9 au BMJ ne pas avoir pris part \u00e0 l\u2019\u00e9tude. La plupart des chercheurs&#x002C; y compris le chercheur principal&#x002C; \u00e9taient injoignables ou ont refus\u00e9 d\u2019\u00eatre interview\u00e9s. De plus&#x002C; plusieurs signataires n\u2019avaient aucune exp\u00e9rience pr\u00e9alable sur la fonction plaquettaire.<\/span><\/li>\n<li><span style='font-family:;' lang='FR-BE'><strong>Probl\u00e8mes m\u00e9thodologiques<\/strong>\u00a0: r\u00e9sultats mal rapport\u00e9s&#x002C; donn\u00e9es manquantes ou incoh\u00e9rentes&#x002C; crit\u00e8res d\u2019\u00e9valuation modifi\u00e9s sans d\u00e9claration&#x002C; et ajustements statistiques non transparents<\/span><span style='font-family:;' lang='NL'>.<\/span><\/li>\n<li><span style='font-family:;' lang='FR-BE'><strong>Probl\u00e8mes de conduite des essais<\/strong>\u00a0: protocole tr\u00e8s contraignant (jusqu\u2019\u00e0 six pr\u00e9l\u00e8vements sanguins en huit heures&#x002C; soit plus de 400 mL de sang par patient sur la dur\u00e9e de l\u2019\u00e9tude)&#x002C; recrutement difficile&#x002C; mesures de base parfois non r\u00e9alis\u00e9es et exclusions non justifi\u00e9es.<\/span><\/li>\n<\/ul>\n<h2><span style='font-family:;' lang='FR'>Transparence des donn\u00e9es de recherche : l\u2019exemple des inhibiteurs de la neuraminidase<\/span><span lang='NL'>\u00a0<\/span><\/h2>\n<p><span style='font-family:;' lang='FR-BE'>L\u2019importance de la transparence est illustr\u00e9e par les \u00e9tudes sur les inhibiteurs de la neuraminidase. En 2014&#x002C; une m\u00e9ta-analyse favorable \u00e0 ces antiviraux publi\u00e9e dans <\/span><i><span style='font-family:;' lang='FR-BE'>The Lancet Respiratory Medicine\u00a0<\/span><\/i><span style='font-family:;' lang='FR-BE'>a \u00e9t\u00e9 contest\u00e9e par le <strong>British Medical Journal<\/strong> pour ses limites m\u00e9thodologiques. Une revue Cochrane fond\u00e9e sur toutes les donn\u00e9es brutes d\u2019essais randomis\u00e9s&#x002C; enfin rendues publiques apr\u00e8s des ann\u00e9es de pression&#x002C; n\u2019a trouv\u00e9 aucun effet sur la mortalit\u00e9 et a montr\u00e9 que leurs b\u00e9n\u00e9fices avaient \u00e9t\u00e9 surestim\u00e9s (voir\u00a0<\/span><a href='https:\/\/www.cbip.be\/fr\/gows\/537'><span style='font-family:;' lang='FR-BE'>Actualit\u00e9s avril 2014<\/span><\/a><span style='font-family:;' lang='FR-BE'> et\u00a0<\/span><a href='https:\/\/www.cbip.be\/fr\/articles\/2237?folia=2195'><span style='font-family:;' lang='FR-BE'>Folia juillet 2014<\/span><\/a><span style='font-family:;' lang='FR-BE'>).<\/span><\/p>\n<h2><span lang='FR-BE'>Le CBIP souligne l\u2019importance de la transparence\u00a0<\/span><\/h2>\n<p><span style='font-family:;' lang='FR-BE'>Le CBIP tient \u00e0 souligner l\u2019importance de la <strong>transparence<\/strong> dans le domaine des \u00e9tudes cliniques.<\/span><\/p>\n<ul>\n<li><span style='font-family:;' lang='FR-BE'>Il est important que <strong>toutes les donn\u00e9es<\/strong> de la recherche soient mises \u00e0 disposition pour que des organisations ind\u00e9pendantes puissent les examiner. L&rsquo;EBM exige naturellement que les donn\u00e9es de recherche issues des essais contr\u00f4l\u00e9s randomis\u00e9s (et d&rsquo;autres \u00e9tudes) soient publi\u00e9es de mani\u00e8re compl\u00e8te et transparente.<\/span><\/li>\n<li><span style='font-family:;' lang='FR-BE'>Les revues doivent \u00e9galement veiller \u00e0 l\u2019exactitude des donn\u00e9es qu\u2019elles publient&#x002C; car il est impossible pour les lecteurs de consulter les donn\u00e9es initiales.<\/span><\/li>\n<li><span style='font-family:;' lang='FR-BE'>L\u2019acc\u00e8s aux donn\u00e9es est \u00e9galement important pour les patients qui participent aux \u00e9tudes cliniques de mani\u00e8re d\u00e9sint\u00e9ress\u00e9e.<\/span><\/li>\n<\/ul>\n<p><span style='font-family:;' lang='FR-BE'>En 2020&#x002C; la Cour de justice de l\u2019Union europ\u00e9enne a pris une d\u00e9cision qui devait <strong>garantir la transparence<\/strong> des rapports d\u2019\u00e9tudes cliniques.
